Initial Presentation (3/2/05)

  • 13 yo WF hx of glasses since 15 mos. of age
  • Vision stable until a few weeks ago
  • Hx: head injury (1 : 1-2-05)
  • Notes gradual ↓ VA (OD>OS)
  • Headaches associated with vomiting daily – been told not to wear glasses (outside optometrist)
  • Allergy: duricef duricef, , ditropen
  • Meds: phenergan phenergan, , Prova
